The County of San Mateo Health System is seeking a Clinical Services Coordinator to join the Health Care for the Homeless/Farmworkers Health Program (HCH/FH), a program within San Mateo Medical Center that is funded by a grant from the U.S. Department of Health and Human Services, Health Resources and Services Administration, under Section 330 of the Public Health Act (covering Community Health Centers). The mission of the HCH/FH Program is to serve homeless and farmworker individuals and families by providing access to comprehensive health care, in particular, primary health care, dental health care, and behavior health services in a supportive, welcoming, and accessible environment. The program is governed by an independent Co-Applicant Board.

The Clinical Services Coordinator is primarily responsible for the monitoring and support of Quality Improvement/Assurance efforts of the program and health policies/protocols for the homeless and farmworker populations, primarily for clinical services. The position is also integrally involved in the development of the program's strategic and tactical plans, mandatory federal reporting, administrative support of the program's governing board (Co-Applicant Board), and preparation of various reports and program documents.
The ideal candidate will have background/training/experience in: clinical settings/patient care, social determinants of health; homeless and/or farmworker programs, particularly health related programs; understanding of health care delivery including outpatient clinics, quality improvement, patient access and case management, EHR (preferably eClinicalWorks (eCW)), and supportive health services; the Patient Centered Medical Home (PCMH) model; strategic planning; supporting a governing board/commission; experience with program data and statistical reporting; and providing training and technical assistance. Additionally, the ideal candidate will:
  • Have a Master's degree in Public Health, Nursing, or related fields and experience in a clinical setting and with underserved populations;
  • Bilingual in Spanish;
  • Knowledgeable in at the areas of:  1) Quality Improvement; 2) clinical protocols; 3) homeless and farmworker health disparities; and 4) social determinants of health;
  • Dedicated to advancing health and equity, as demonstrated through work experience and/or personal experience having been adversely affected by health inequities;
  • Familiar with interpreting Federal policies, budgeting and contract management;
  • Able to build and establish relationships, and acts as a liaison, between HCH/FH and SMMC clinics & programs, SMC Health System programs, and other hospitals, clinics, and health care services to address the medical needs of the homeless and farmworker population in San Mateo County;
  • A creative thinker and problem solver;
  • Experienced with community engagement, collaboration and partnerships;
  • Experienced in maintaining relationships with advocates, community partners and government staff;
  • Experienced with providing training and technical assistance in a clinical setting;
  • Politically astute and able to represent HCH/FH at public meetings;
  • Excellent in oral and written communication, including developing and writing reports, creating presentations and public speaking.

This is a limited term, at-will position.  Limited Term Employees are assigned to assignments not to exceed three years. They receive medical and dental benefits, similar to regular employees, and a defined contribution retirement plan.  Examples Of Duties:
  • Provide technical, programmatic and/or clinical assistance to staff, subcontractors and management, including the development of program policies and procedures and the training of staff.
  • Compile and maintain accurate records and files regarding program activities.
  • Evaluate program effectiveness against program goals, funding source or contract requirements and other regulations; identify program problems, present findings and recommend appropriate action.
  • Collaborate with management and program staff to write policies and protocols for homeless and farmworker patients.
  • Acts as liaison with internal (SMC) and external medical care resources developing linkages to promote improved patient care and encourage support for homeless & farmworker health care, including non-medical services
  • Act as liaison with community-based organizations, service providers, County departments, State and/or other funding sources, commission, planning council and advisory board in support of assigned program/s.

  • Participate in departmental, Countywide and/or State planning process.
  • Serve on variety of committees and task forces related to assigned program(s). Write grant proposals, reports, program documentation, policies, procedures and other written materials.
  • Analyze and incorporate legislative changes and other relevant information into program policies.
  • Coordinate and conduct special events and projects.
  • Perform related duties as assigned.

  • Establish and maintain effective working relationships with grantors, subcontractors, County staff, elected and appointed officials, the public and others.
  • Train staff in work procedures and provide technical and programmatic assistance to staff and subcontractors.
  • Communicate effectively, both orally and in writing.
  • Exercise sound independent judgment within general policy and regulatory guidelines.  

Education and Experience:
Any combination of education and experience that would likely provide the required knowledge, skills and abilities is qualifying.  A typical way to qualify is:

Community Program Analyst II: Bachelor's degree in public or business administration, management or a related field and four years of professional level analytical, administrative and programmatic experience in a community-related setting, including some experience in contract monitoring.
