HEALTH PLAN OF SAN MATEO

to San Mateo County’s underserved population. The Plan currently serves approximately 147,000 county
residents. We are deeply committed to ensuring that high-quality care and services are accessible and
affordable for all our members. Each department within HPSM plays a key role in advancing our work

related to meeting quality standards. Our three-year goals and strategies focus on continually improving
our services and programs, strengthening our partnerships with health care providers, and integrating
services across public and private systems of care. HPSM has a staff of 29 members, including 9

throughout the year informed by the Behavioral Health Integration grant.

CalAIM: We will implement programs and operational structures to prepare for the next phase of state
transformation, which may include new models of care coordination, member outreach, and long-term services and
      supports (LTSS).
Justice, Equity, Diversity and Inclusion: We will embed policies and practices in our organizational culture and
operations to promote a more inclusive workplace that fosters hiring, retention, and career paths for historically
      underrepresented communities.

      planning and decision-making.
